1. Use Shift + Tilde to open the console.
2. Type /connect multitap.pw
3. Alternatively, look for “Burton Gilliam Appreciation Server” in the server list.

1. Play duel mode (or possibly other modes where you can die/spectate as a fly)
2. Die and spectate (conflicting reports on whether you need to die or become a spectator after winning)
3. Join the game again
4. You've created a holoduke
